We agree to pay $60 dollars toward every RPM to Picture Perfect Health, LLC on the 20th of each month on paid services only from Medicare and acceptable Medicare advantage plans, via check or credit card.
This contract is non-cancellable for 1 (one) year and auto renewable every 36 months, and requires 90 days advanced notice to cancel, before the end of 1 year period.
